This form is used by an applicant for naturalization to request that the Department of Defense verify the applicant's military or naval service.

Return the form to the office which requested the certification.

Special Instructions: 

Pages 1, 3 and 5 of the form are identical and are to be filled out by the applicant.
Pages 2, 4 and 6 are to be filled out by a certifying officer of the Department of Defense.
